CRUSADERS CONSTITUTION

CRUSADERS GLIDING CLUB

APPLICATION FOR TEMPORARY MEMBERSHIP AND TRIAL LESSON

1 .             I am interested in gliding and hereby make application to the Club for 28 days' temporary membership and a trial lesson.  I am over the age of 18 years (This form must be signed by the parent or guardian of persons who are not yet 18).

2 .             I hereby accept that myself and my property are entirely my own responsibility while I am participating in Club activities.  I agree that all members of the Club, and the Club itself, including its servants and agents will be under no liability whatsoever arising out of my participation in Club activities or my use of Club property.

3 .             I agree to abide by the rules of the Club, as published in the Constitution or elsewhere, and to obey any safety or flying orders which may be given to me by any member of the Club whether these orders are verbal or written.

4 .             I sign the form below in acknowledgement.
