I apply to become a member of Crusaders Gliding Club and agree to pay subscriptions as due in accordance with current rates. I undertake to abide by the rules of the Club and I will take particular care: 
  
To read and sign the Flying Order Book. 
  
To produce a certificate of medical fitness before flying solo. 
  
Not to attempt to carry out any task unless I have been briefed by a competent Club member. 
  
Data Protection Act. I supply personal information as required on page 2 of this application and agree that it may be stored on a computer data base and used as governed by the Act in connection with gliding administration by the Club and by the governing bodies of the sport.
Indemnity. I hereby indemnify the Club and the Ministry of Defence against loss or damage to my personal property and personal injury, including death, resulting from any negligence, act or omission on the part of any member or visitor. 

Consent Form. My son/daughter/ward, being under the age of 18, I hereby consent to him/her participating in the activities, including solo flying, of Crusaders Gliding Club:
